|
Aftercare means that clients continue to see their keyworkers, usually on a weekly basis and receive support on a wide spectrum of issues including alcohol, accommodation, family, supports, legal, debt and health. Clients on aftercare still work to care plans that are reviewed with their keyworkers and also Care Manager if they have not closed the case. It is reviewed with the client and support worker regularly to see if the aims have been achieved and to look at what the aims of the next care plans are. Aftercare may be withdrawn if a client does not carry out the aims in their care plan. Most of the aftercare sessions take place within Albyn House, however keyworkers will, at times, visit the clients in their home.
